New/Returning Student Registration

In order to register a student at Central Valley School District, you will need to schedule an appointment. To schedule, please call 724-775-5600 extension 13010 to speak to the central registration office or you can send an email to enroll@centralvalleysd.net

Appointments are to report to the South Entrance of the Central Valley High School. The Central Registration Office is located at 160 Baker Road Extension, Monaca, PA 15061.

REQUIRED DOCUMENTS

The following documents are required to register a student with the Central Valley School District: 

  • Birth Certificate
  • Proof of Residency - Residency must be verified to be in the Central Valley School District by providing any combination of the following documents that includes one (1) document from Category A AND one (1) document from Category B:
    • Category A- one (1) document:
      • Current lease/rental agreement
      • Current mortgage agreement
      • Residency affidavit from resident family member or friend (Parental Registration Statement - Attachment B and C)
    • Category B - one (1) document:
      • Current utility bill (i.e., water, gas, electric, cable, telephone)
      • Most current tax record (within one (1) year)
      • Payroll stub (within sixty (60) days)
      • Bank or Credit card statement (within sixty (60) days)
      • W-2 form (within one (1) year)
      • Post Office change of address form
  • Immunization Record
  • Valid PA Driver's License or Valid PA Photo Identification card
  • Proof of Guardianship (if applicable)
